5. One passport size colour photograph each of the parents
6. Photocopy of Birth Certificate
7. Photocopy of Report Card of Class IX
8. Medical Certificate
9. Registration Card and Registration Slip of Class IX
10. Parent’s Transfer Letter
11. Affidavit of Old and New Address Proof
